TRANSFER/CANCELLATION OPTION*

I accept that on the notified date of transfer I'm authorising the cancellation of the existing alarm monitoring service to self notification service ONLY (NON MONITORED).

*TRANSFER OF OWNERSHIP CONSENT*

I consent to these transfer of ownership conditions:

I accept that on the notified end of service date that in effect I'm authorising the cancellation of and transfer of the existing service to the new owners. Any outstanding amounts owed up to and including the end of service date will be reconciled and charged to the existing client as per MSE Security TOT.

*CANCELLATION AUTHORISATION*

I consent to these cancellation conditions:

I'm authorising the cancellation of the alarm monitoring as of the notified date of Cancellation. I accept there is a 30 days notice of cancellation required and a Charge for $220 + GST for the cancellation and decommissioning of the system. I also acknowledge any outstanding amounts owed will be reconciled and charged to the existing client as per MSE Security TOT.

*CANCELLATION + COMMUNICATOR RETURN*

I consent to these cancellation conditions:

I'm authorising the cancellation of the alarm monitoring as of the notified date of Cancellation. I have arranged to remove and return the Alarm Communicator and return it to MSE Security Pty Ltd with 7 Days OR in the event I fail to return the Communicator within the 7 day period I authorise the Charge of $220 + GST. I accept there is a 30 days notice of cancellation required and any outstanding amounts owed will be reconciled and charged to the existing client as per MSE Security TOT.

During business hours on the date of transfer the alarm monitoring service will be cancelled and the service transferred to Self Notification Service Only (NON Monitored) .

*Date of Transfer*

During business hours on the date of transfer the alarm monitoring service will be transferred to the control of the new owner/tenant.

*Date of Cancellation*

During business hours on the date of cancellation the alarm monitoring service will be terminated.
